1) ContinUCare:
In addition to our aftercare programming, we offer ContinUCare.
Exclusive to Trafalgar Addiction Treatment Centres, ContinUCare is an empirically and clinically-based system, supported by technology. It is designed to maintain an ongoing, individualized link between you and your Trafalgar care team. ContinUCare is an advanced, highly effective complement to traditional Aftercare Support Groups.
ContinUCare is available to all Trafalgar clients at no additional cost.
2) Continuation of Care:
Continuation of Care Program Supports You Beyond Structured Treatment.
Our Continuation of
Care counselling is unique among Canadian treatment providers. You meet with our Continuation of
Care Counsellor during treatment to establish a relationship that will support your lasting recovery. You then
schedule biweekly calls with our Continuation of Care Counsellor for four months after graduating from addiction
treatment. This calls will follow monthly for 20 months after the 4-month period.
These calls allow you to identify and address any challenges you are facing in your recovery with the support of our clinical team. The program ensures that our support for you continues beyond the completion of a structured treatment program.
Continuation of Care is available to all Trafalgar clients.
